The content below this paragraph shows how Survey Builder will appear to your administrators. The example shows five questions available. Because of some of the colorful questions that have been entered, this page is now a demo only. Click continue to see the how the Survey Builder works, if required fields are not entered, error messages will display. If all required fields are entered, and at least one question type is not 'Text Only', page two, the page to enter answers will display.

You can add or modify your survey questions by using the edit fields below. Setting up your survey questions is done using two pages of information.

On this first page you will enter the question, the question type, and the number of answers. On the second page you will enter the answers to the questions that weren't type 'Text Only'.

On this first page you will need to enter the following information for each question in the edit boxes below.

The Question Type:

Text Only – these are questions that only require a text answer.
Single Answer – yes or no; true or false; or only one choice out of list of multiple choices
Multiple Answer – the user can select more than one choice out of list of multiple choices

The Number of Answers:

Example #1: A question with an answer of “yes” or “no” would have two (2) answers.
Example #2: A question with five multiple choice answers, where only one answer could be selected, would have five (5) answers.
Example #3: A question with ten multiple choice answers, where more than one answer could be selected , would have ten (10) answers.

The Question:

Record the question you are asking in this edit box.

Once you are satisfied with the questions you have prepared, click the ‘Continue’ button to go to the second survey setup page if you have selected any single or multiple answer questions. On the second page you will enter the answers to those single answer and multiple answer survey questions.
